To help your appointment move along promptly and
smoothly, and to ensure your enrollment on the same day,
please bring the following items with you when you arrive
at PPGI for your appointment:

• Proof of citizenship such as a birth certificate or passport
• Non U.S. citizens need to bring immigration documents
• Completed form (if you have not already submitted
  it online or returned it in person or by mail)
• Photo identification
• Social Security Number (bring your card or know
  your number)
• Proof of income from past 30 days:
  - Pay stub (at least one) or,
  - Statement from employer or,
  - Unemployment benefits or,
  - A tax return if you’re self-employed
• Proof of child care costs
• Proof of child support either paid or received
  (payment record or court order)
• Proof of alimony either paid or received
  (payment record or court order)

Even if you don’t have these documents, please keep your appointment with PPGI.
