PPGI will review your form and other paperwork to
determine your eligibility.

If you qualify for PlanSmart:

• You can receive free birth control services on the
  same day you apply – with no delay;
• or, a notice of the decision will be placed in your
  chart and given to you at your next visit.
• You will receive an insurance card at the address
  you provide. (You can either have the card sent to
  your local PPGI center or to a specified address –
  it’s your choice.)

If you don’t qualify for PlanSmart:

• You can still receive services at PPGI and may
  qualify for a discount.
